The Local Coverage Article "Standard Documentation Requirements or All Claims Submitted to DME MACs" (A55426) provides details about documentation on prescriptions, medical documentation, and proof of delivery.

 All DMEPOS claims require a prescription.

The prescription is also known as an "order" or "Standard Written Order."

The physician signing the prescription is listed on the claim, along with the physician's NPI.

According to Medicare, the following may prescribe DMEPOS items:

  • Doctor of Medicine (MD),
  • Doctor of Osteopathy (DO),
  • Doctor of Optometry (OD),
  • Doctor of Medical Dentistry (DMD),
  • Doctor of Dental Surgery (DDS),
  • Doctor of Podiatric Medicine (DPM),
  • Physician assistants (PA), nurse practitioners(NP) and clinical nurse specialists (CNS). NOTE: the scope of practice is determined by state law.

As stated in the article, chiropractors are not permitted to prescribe DMEPOS items.

Signature stamps are not allowed. The physician's signature must be legible and authenticated
