According to Medicare, the treating practitioner is a physician, a physician assistant, a nurse practitioner, or a clinical nurse practitioner.


Prior to dispensing any DMEPOS items, a prescription must be received. For Medicare purposes, the prescription is called “Standard Written Order.” The SWO must be received from the “treating practitioner” prior to claims submission. According to Medicare, the treating practitioner is a physician, a physician assistant, a nurse practitioner, or a clinical nurse practitioner.

The Medicare Guidance Manual provides additional information about what specialties are covered under the definition of physician. A doctor of medicine, a doctor of osteopathy, or a doctor of podiatry may sign the SWO. All must be licensed to practice within the state of service.

The SWO may be created by someone other than the treating practitioner. The treating practitioner must review and sign the document.
