Proof of Delivery

A DMEPOS supplier is required to maintain a proof of delivery (POD) document in the patient’s files. The POD must be available upon request. To be compliant, the following elements must be included on the POD:

  • Patient’s Full Name
  • Patient’s Address
  • Date of Delivery
  • Delivery Address
  • Quantity and description of items being provided. (This can be a narrative, a HCPCS code, the long description of the HCPCS code or a brand name product and model number.)
  • Signature of patient or patient’s representative

The date of delivery becomes the date of service (DOS) on the claim. If the POD has a supplier entered delivery date and a patient’s signature date, the patient’s signature date is the date of service.

What Does This Mean for You?

Most practice management software offers POD templates. Review yours to make sure you have all the necessary elements.

Double check POD for matching dates. The date the patient signs is the date of service on your claim.

Maintain a copy of the signed POD in the patient’s file.
