What is a Letter of Medical Necessity?

A Letter of Medical Necessity (LOMN) is a tool used to communicate information needed to request authorization of a device. The information in the letter describes the reason this device will effectively meet the needs of the patient.

An effective LOMN includes information about the patient's pertinent medical history, current condition, rehabilitation potential and an explanation of how this device will benefit the patient.

It is important to understand the payer's definition of medical necessity and document how this device is medically necessary for this patient. The letter may also include a statement indicating there is no equally effective device which is less costly.

The LOMN must be on the clinic's letterhead, signed by a healthcare professional, and dated. Remember, the LOMN is not considered to be part of the patient's medical record. So all information in the LOMN must be supported in chart notes, medical records, and reports.
