If submitting CAD files, please print out the Custom Knee Brace Order Form, fill it out by hand and fax it to Össur. E-mail the CAD files as attachments to: cadorders@ossur.com If you wish to submit a cast with your order, please complete the Custom Knee Brace Order Form and send both order form and cast to:

Casting Instructions:

• Make a cylinder cast 12" proximal and 12" distal to mid-patella, using plaster or fiberglass 
• Patient should be cast non-weight bearing with knee in full extension and ankle at 90° 
• Circle patella, fibular head, and mark joint line 
• Use stockinette only 
• Do not use cast padding
